What services do you provide?
We are a primary health care provider for registered patients. We are not a walk-in clinic. Please make an appointment with our Health Care team.

How do I become a patient?
To make your search for a family physician a little bit easier the Government of Ontario has created Health Care Connect. If you are currently without a family doctor you can register by calling the toll free number 1-800-445-1822. You can also visit Health Care Connect

From time to time there may be openings for new patients at the East End Family Health Team; however, there is no waiting list for new patients. The best way to discover if you are able to become an East End patient is to contact our receptionists and inquire about our current patient enrolment.

How do I renew my prescription(s)?

We have updated our prescription renewal process to make it easier for our patients. It is now a ONE step process:

  • Call your pharmacy and advise them you need a medication renewal
    • The pharmacy will contact us with all the information we need to processes it for you.

Where are you located?
Our address is 4715 Highway 101 East, South Porcupine Ontario, P0N 1H0. We are located at the Northern College, Timmins Campus. See our Google Map for directions.

NOTE: The entrance for the East End Family Health Team is located at the west end of Northern College and not through the main entrance for the college. The East End Family Health Team Clinic has a separate entrance than the college.
